In a study comprising of two 13-day human laboratory inpatient admissions, lorcaserin 10 mg BID was found to decrease cannabis self-administration in 15 non-treatment seeking individuals with daily use, compared to placebo. Lorcaserin also decreased craving during abstinence conditions, and co-morbid tobacco smokers decreased their tobacco use, even though they were not intending to (34). However, to be diagnosed with a substance use disorder, a person must meet specific diagnostic criteria for continued substance use despite negative consequences. Taking an adulterated drug can lead to unexpected and unwelcome side effects and may increase its potential health risks. Over the past decade, illegally made opioids like fentanyl have been increasingly found in the drug supply, and have contributed to a dramatic rise in drug overdose deaths in the United States.
Acute and chronic psychosis is an adverse effect of particular concern, as exemplified in the case study reviewed above. A systematic review by Studerus et al. including psychedelic-assisted therapy studies from 1999 to 2008 noted that among participants receiving psilocybin, 27% experienced fear and 17% paranoia. In other studies, 7% of subjects in the highest dose conditions fit the criteria for acute psychotic reactions. These events were confined to the acute phase and were managed by interpersonal support.
Multiple testing can increase Type I Error rates, so we applied a Bonferroni correction when we interpreted results. However, Bonferroni corrections are conservative so we also present unadjusted p-values to allow readers recovery games for groups to see the impact of adjustment and consider other adjustments less conservative than Bonferroni’s correction. It should be noted that the majority of significant findings remain significant after applying the correction.
The therapeutic effects from these experiences can be enduring, and it is conceptualized that during the psychedelic experience a therapeutic window in the mind is temporarily opened which facilitates gained insight and emotional release. In conjunction with psychotherapeutic support, this insight can potentially lead to a healthy revision of outlook and lifestyle (47). The spatial discrimination component of the double Y-maze requires the use of reference memory only, whereas the delayed alternation component also requires the use of working memory (Mallet and Beninger, 1993). Our findings with THC are therefore in good agreement with previous studies (eg Mallet and Beninger, 1996, 1998; Nakamura et al, 1991), showing that THC impairs working, but not reference, memory. At low to moderate doses, the memory-impairing effects of THC and THC+MDMA were specific to the delayed alternation component of the task; that is, the spatial discrimination component was unaffected.

Attraction to others was similar, with the most participants (72.3%) reporting alcohol use as leading them to be more attracted to others, followed by ecstasy (64.3%), and then marijuana (27.0%). Increased social outgoingness was reported by the majority of users of alcohol (77.1%) and ecstasy (72.3%), yet only a quarter (26.1%) of participants reported that this increased when using marijuana. The neurochemical basis for the observed synergistic effect of the combined drugs is an interesting matter for speculation. Evidence regarding direct interaction of cannabinoid and serotonergic systems in impairment of memory is very limited (Egashira et al, 2002; Molina-Holgado et al, 1995). MDMA has direct action on the serotonin, dopamine, and norepinephrine neurochemical systems (Climko et al, 1986), suggesting that an interaction of the two drugs may occur within the dopamine system.
As described in a retrospective observational study by Cox et al., psychedelics that target 5HT2A receptors appear to reduce the consumption of cannabis (28). Furthermore, when 5HT2A receptors are directly acted upon, they appear to enhance psychological domains noted above, such as insight, self-efficacy, and spirituality, suggesting potential for targeting serotonergic receptors for CUD treatment (29). Substance abuse and sexual education programmes should address harm reduction related to having sex while using psychoactive substances. These programmes may help inform substance users about the effects of these substances on sexual experiences.
Most of MDMA-related deaths occur in individuals in their early teens to mid-20s and who are from middle and upper middle class families (DEA, 2001). In the United Kingdom, where about 4.5% of young adults aged 15–34 years have taken MDMA on at least one occasion in the previous 12 months (Hall and Henry, 2006), Schifano et al. (2006) reported 394 related fatalities between 1994 and 2003. In 42% of these cases, the cause of death was MDMA alone; in all other cases, co-factors contributed. In 2002, MDMA represented slightly more than 4% of the drug-related fatalities documented in the United Kingdom. The endocannabinoid system (ECS) has a crucial role in the neuromodulation of rewarding and neurophysiological actions by drugs of abuse. A suite of lipid neurotransmitters, their synthetic and catabolic enzymes, and their preferential cannabinoid receptors (CB1, CB2), the ECS modulates release of numerous other neurotransmitters and therefore offers a key contribution to learning and behavioral responses.
The present study therefore aimed to investigate the combined effect of acute administration of MDMA and THC on a major component of cognitive function, namely memory, in rats. Specifically, it sought to document the effects of the two drugs co-administered, across a range of low to moderate doses that are representative of human use (Boot et al, 2000). 3,4-Methylenedioxymethamphetamine (MDMA, ‘Ecstasy’) and cannabis are two of the most commonly used illicit drugs in the western world, and are often used in combination. Very little research has examined their effect on cognitive function or behavior when combined.
